Location illustration for abdomen subcutaneous injection

Abdomen

Choose fatty tissue at least 2 inches (5 cm) from the navel.

Location illustration for side of abdomen subcutaneous injection

Side of abdomen

Choose healthy fatty tissue below the ribs and above the hip bone.

Location illustration for back of upper arm subcutaneous injection

Back of upper arm

A trained helper can use the fatty tissue on the back of the upper arm.

Location illustration for outer thigh subcutaneous injection

Outer thigh

Choose fatty tissue on the outer middle thigh, away from the knee and hip.

45° and 90° examples

Medical cutaway showing a 45-degree needle ending in subcutaneous fat above muscle
45° can be used with some needles or a thinner fat layer.
Medical cutaway showing a perpendicular SQ needle ending in fat above muscle
90° can be used with a short SQ needle and sufficient fatty tissue.

These illustrations identify tissue layers and example angles; they are not to scale. Needle length, angle and whether to lift a fold depend on the prescribed device and your training; stop and ask the clinic if either is unspecified.

  1. Check the order and equipment

Match name, vial strength, final concentration, SQ route, expiration and discard date to your prescription. Use the prescribed U-100 syringe only if the order specifies it; its marks measure volume.

  2. Prepare and measure

Wash and dry hands. Use a clean surface, a new sterile needle and syringe, and an alcohol pad. Check solution appearance, clean the stopper and let it dry. Draw only the prescribed volume and recheck the mark and bubbles.

  3. Choose and clean a site

Choose one healthy site shown above and rotate each time. Avoid red, hard, bruised, scarred or tender skin. Clean the spot and let it dry. Do not touch it again.

  4. Inject into fatty tissue

Gently lift a skin-and-fat fold if your training calls for it. Use the 45° or 90° angle specified for your needle and body habitus. Insert into fat, press the plunger steadily, then withdraw at the same angle.

  5. Finish and dispose

Press clean gauze if needed; do not rub. Do not recap or reuse the needle. Put needle and syringe immediately in a sharps container; close it at about three-quarters full and use local disposal rules.

Missed dose or error

Do not double, repeat, or titrate without the written order. Contact the clinic for a discrepancy, uncertain storage, or unexpected reaction; urgent symptoms need emergency care.

Refills and switching
